Put your nursing, psychology or social work care management skills to good use……… Partners Health Plan, Inc., (PHP), a growing company, dedicated to people with intellectual and developmental disabilities seeks compassionate licensed and highly organized RN, Psychology or Social Work Care Managers to provide support in a team-based environment that is mission driven, results oriented and sensitive to individuals with intellectual and developmental disabilities. The Care Manager is a team leader responsible for supervising assigned Care Coordinators in overseeing the full continuum of activities for individuals with intellectual and developmental disabilities, including assessments, care and service planning and ongoing monitoring and coordination of care for PHP’s plan participants. Opportunities are available in these PHP chapter location: · Westchester, NY Successful candidates will possess a current license and or registration to practice in the state of New York, such as a Licensed Psychologist, Social Worker or Registered Nurse (RN) along with a minimum of one (1) year of work experience with persons with intellectual and, or other developmental disabilities required. A minimum of an Associate’s degree in a health-related field is also a must. Bachelor’s degree preferred. Excellent interpersonal, communication and organizational skills along with attention to detail are also required. A working knowledge of Microsoft Word and Excel along with proficiency with health-related computer applications desired. Knowledge of the Willowbrook injunction as it applies to Willowbrook Class Participants also preferred but not required. Ability to travel as necessary also required with a valid driver’s license to travel in New York State preferred. ​
